چکیده
INDIAN PEDIATRICS 1094 VOLUME 43__DECEMBER 17, 2006 An 8-year-old boy diagnosed as Glanzmann thrombasthenia (GT) by other center was referred to our department in March 2005. He has been followed with GT for 4 years. He complained difficulty in breathing and recurrent upper respiratory tract infections. We observed a big perforation at nasal septum and granulation tissue related with forgotten nasal tampon on nasal examination. Results of complete blood count were as follows: hemoglobin 8.9 g/dL, mean corpuscular volume (MCV): 64 fl, white blood cell count 4800/mm3, and platelet count 321000/mm3. He was hospitalized for cleaning of the granulation tissue. In our patient alloimmunisation had developed due to frequently platelet transfusion to stop resistant nasal and gingival bleedings before the admission. The patient was operated with local anesthesia. Forgotten tampon was removed and granulation tissue was resected. Regeneration of septum nasi was performed. Bolus injection of Recombinant activated factor VIIa (rFVIIa) 90 μg/kg was given immediately before operation and three times with 2 hours intervals after the surgery. The patient was discharged one day after without any complication.

The international, prospective Glanzmann Thrombasthenia Registry: treatment and outcomes in surgical intervention.
Standard treatment for Glanzmann thrombasthenia, a severe inherited bleeding disorder, is platelet transfusion. Recombinant factor VIIa is reported to be effective in Glanzmann thrombasthenia with platelet antibodies and/or refractoriness to platelet transfusions. We aimed to evaluate recombinant factor VIIa effectiveness and safety for the treatment and prevention of surgical bleeding in patie...
متن کاملPyelolithotomy in a patient with Glanzmann thrombasthenia and antiglycoprotein IIb/IIIa antibodies: the shortest possible duration of treatment with recombinant activated factor VII and platelet transfusions.
Transfusion of platelet concentrates remains the first-line therapy for Glanzmann thrombasthenia in case of bleeding or preparation for surgery. However, development of antibodies to platelet glycoprotein (Gp) IIb/IIIa complex or human leukocyte antigens (HLA) is frequent and the main cause of platelet refractoriness. Recombinant activated factor VII (rFVIIa) is a potent alternative for patient...
متن کاملRecombinant factor VIIa is effective for bleeding and surgery in patients with Glanzmann thrombasthenia.
Recombinant activated factor VII (rFVIIa) was found to be effective and safe in treating 24 bleeding episodes and to prevent bleeding during one bilateral herniorrhaphy in four children with Glanzmann thrombasthenia. One of the patients had alloantibodies to platelet membrane glycoprotein (GP) IIb/IIIa and was refractory to platelet transfusion. rFVIIa was administered at 89 to 116 microg/kg pe...

متن کاملIntravenous regional analgesia in a patient with Glanzmann thrombastenia.
Glanzmann thrombastenia (GT) is a rare condition of an inherited autosomal recessive gene characterized with bleeding tendency. The condition is rarely met in the OR. and therefore it is essential that anesthesiologist be cognizant of the risk involved and be prepared with all necessary precautionary measures. We present a GT case in a 27-year-old male with a mass in the anticubital region of r...
